“I have been a Social Worker for 12 years having patients from 35 years old all the way up to 90. I had a one patient, a 50-year-old man with life-long mental health issues who lived in a very small Florida town. He was having difficulty finding resources in his area – he needed a therapist, assistance with Medicaid services and transportation. I helped him find the community resources he needed, some of which he wasn’t aware existed, to help him meet his goals. And that’s my goal, to give patients the tools they need to get where they want to be. To do that, it is important to have empathy and see where the patient is coming from. You can’t be judgmental; you have to meet them where they are.” 


Our patient-centered care model emphasizes holistic treatment plans - taking into consideration physical, financial and environmental factors. As a social worker, Danielle connects providers and patients with the resources needed to ensure all facets of a patient's healthcare plan are met.
